Job summary

Resources for Human Development (RHD) is seeking a compassionate Case Manager to coordinate care for youth and families in Wilmington, DE. This is a part-time role focused on maintaining treatment plans and linking services in a collaborative, person-centered environment.

You will work with clinicians and families, maintain records, and participate in ongoing training. The position offers flexible shifts, competitive pay, and growth opportunities within a nationwide network.

Qualifications

  • Bachelor's degree and one year of related experience preferred, or an equivalent combination of education and experience
  • Ability to establish a HIPAA-compliant home office
  • Valid driver's license
  • Ability to pass drug screening and state and federal background checks

Responsibilities

  • Coordinate medical care and treatment services for youth in care
  • Maintain medical records and ensure regulatory compliance
  • Schedule and coordinate routine, specialty, psychiatric, and follow-up appointments
  • Assist with transportation to medical appointments as needed
  • Monitor medication administration and complete weekly medication counts
  • Train staff in medication administration, CPR, and First Aid procedures
  • Communicate medical updates and treatment information with parents, guardians, providers, and treatment team members
  • Collaborate closely with clinicians, direct care staff, families, and community partners to support positive outcomes
  • Maintain accurate documentation and electronic records
  • Assist with program operations and provide support coverage when necessary
  • Support quality care initiatives and contribute to overall program success
  • Perform additional duties as assigned

What You'll Do

  • Part-Time
  • Friday: 12:00 AM to 8:00 AM
  • Saturday: 12:00 AM to 8:00 AM
  • Additional weekday hours may be assigned to meet full-time status
  • Occasional flexibility may be required based on program needs
Physical Requirements
  • Ability to lift up to 25 pounds
  • Ability to stand, walk, bend, and assist youth as needed
  • Ability to perform essential job functions with or without reasonable accommodation
