In this role, you will serve as an advocate, coordinator, and trusted support for participants by connecting them to resources, developing individualized service plans, and collaborating with community partners to help them reach their goals.

The Case Manager is responsible for assessing participant needs, coordinating and implementing individualized service plans, and advocating to ensure access to necessary services and resources. Through a recovery-oriented and person-centered approach, the Case Manager helps participants achieve their goals, improve their quality of life, and successfully navigate community systems and supports.
Working collaboratively with participants, families, interdisciplinary teams, and community providers, the Case Manager plays a vital role in promoting positive outcomes and long‑term success.
Ability to communicate effectively and professionally with participants, families, providers, and community partners.
Ability to evaluate participant needs, identify barriers, and determine appropriate interventions and resources.
Strong organizational skills with the ability to manage multiple participants, service plans, and community partnerships.
Ability to work effectively within multidisciplinary teams and maintain productive working relationships.
Commitment to supporting participant rights and ensuring access to needed services and resources.
Ability to identify challenges, develop solutions, and respond effectively to changing participant needs.
Understanding of community resources, public benefits, healthcare systems, housing supports, and recovery services.
Ability to maintain accurate records and documentation while meeting regulatory and funding requirements.
