Job summary

Resources for Human Development (RHD) in Bethlehem, PA, is seeking a dedicated Case Manager for its Lehigh Valley ACT Program. The role focuses on delivering recovery-oriented, community-based services within a multidisciplinary ACT team to help participants achieve meaningful independence and wellness.

You will connect individuals with essential resources, advocate for needed supports, and document progress while collaborating with partner agencies across the Lehigh Valley.

Qualifications

  • Two years of experience supporting individuals in behavioral health, addiction recovery, homeless services, or BHH/IDD programs
  • Intermediate computer skills and ability to complete electronic documentation
  • Strong interpersonal, communication, and customer service skills
  • Ability to work independently and as part of a multidisciplinary team
  • Valid driver's license and ability to travel throughout the community as required
  • Bachelor's Degree in Human Services or a related field (preferred)
  • One year of case management experience (preferred)
  • Experience working with individuals experiencing behavioral health challenges (preferred)
  • Knowledge of community resources and recovery-oriented services (preferred)

Responsibilities

  • Work as part of a multidisciplinary ACT team providing holistic, recovery-focused services to participants
  • Assist individuals in developing and achieving goals outlined in their recovery plans
  • Connect participants to community resources, educational opportunities, healthcare services, and supportive programs
  • Advocate for participants to ensure access to needed services and supports
  • Develop and maintain relationships with community agencies and service providers
  • Provide community-based services throughout the Lehigh Valley service area
  • Transport participants to appointments and community activities when needed
  • Complete assessments, service documentation, and reports in accordance with organizational and regulatory requirements
  • Identify service gaps and recommend strategies to enhance participant outcomes
  • Support participants in achieving greater independence, stability, and quality of life

What You'll Do

Schedule
  • Full-Time Position
  • Community-based work throughout the Lehigh Valley region
  • Schedule flexibility required based on participant and program needs
  • Some evening, weekend, and location flexibility may be required
Physical Requirements
  • Ability to lift up to 50 pounds occasionally
  • Ability to sit, stand, walk, bend, reach, kneel, crouch, and climb stairs as needed
  • Ability to travel regularly throughout the community
  • Ability to perform essential job functions with or without reasonable accommodation
