Resources for Human Development in Norristown, PA seeks a dedicated Case Manager to support youth and families through coordinated medical care, records management, and collaboration with clinicians and caregivers.

You will coordinate appointments, monitor medications, and train staff while ensuring regulatory compliance and high-quality, compassionate care.

Qualifications

• High school diploma or equivalent required.
• Strong written and verbal communication skills.
• Compassionate, organized, dependable and team-oriented.
• Interest in behavioral health and case management.

Responsibilities

• Coordinate medical care and treatment services for youth in care.
• Maintain medical records and ensure regulatory compliance.
• Schedule and coordinate routine, specialty, psychiatric, and follow-up appointments.
• Assist with transportation to medical appointments as needed.
• Monitor medication administration and weekly counts.
• Train staff in medication administration, CPR, and First Aid.
• Communicate updates with parents, guardians, providers, and team members.
• Collaborate with clinicians, direct care staff, families, and community partners.
• Maintain accurate documentation and electronic records.
• Assist with program operations and coverage when necessary.
• Support quality care initiatives and overall program success.
• Perform additional duties as assigned.

What You'll Do

• Coordinate medical care and treatment services for youth in care
• Maintain medical records and ensure compliance with regulatory requirements

• Train staff in medication administration, CPR, and First Aid procedures
• Communicate medical updates and treatment information with parents, guardians, providers, and treatment team members
• Collaborate closely with clinicians, direct care staff, families, and community partners to support positive outcomes
• Maintain accurate documentation and electronic records
• Assist with program operations and provide support coverage when necessary
• Support quality care initiatives and contribute to overall program success
• Perform additional duties as assigned
