Job summary

Minority Veterans of America (MVA) seeks a dedicated Case Manager to support veterans in our Transitional Housing Program and MVCC services. You will manage a small caseload, conduct assessments, and develop housing stability plans.

This full-time, in-person role involves on-site days, plus one remote day for documentation; you will coordinate referrals to healthcare, benefits, and community resources, and maintain thorough case notes.

Qualifications

  • Bachelor's degree in Social Work, Human Services, Psychology, or related field, or equivalent experience.
  • Experience providing case management to veterans, individuals experiencing homelessness, or other vulnerable populations.
  • Knowledge of VA healthcare, VA benefits, housing programs, and community resources.
  • Strong organizational, documentation, and time management skills.
  • Ability to work independently while collaborating effectively with hybrid/remote team.
  • Experience with trauma-informed care, harm reduction, motivational interviewing, and strengths-based case management preferred.

Responsibilities

  • Provide ongoing case management for Transitional Housing residents.
  • Conduct comprehensive assessments to identify strengths, barriers, and service needs.
  • Develop, implement, and regularly update individualized housing stability and service plans.
  • Meet with residents regularly to monitor progress toward goals.
  • Guide residents in attaining permanent housing and housing stability.
  • Coordinate referrals for healthcare, behavioral health, substance use treatment, employment, education, legal services, and other resources.
  • Assist residents with benefits applications, including VA benefits and public assistance.
  • Maintain accurate case notes, assessments, and program documentation.
  • Monitor participant compliance with program expectations and provide supportive interventions.
  • Identify ways for residents to engage as a household.
  • Participate in case conferencing and community partner meetings as needed.
  • Respond to participant crises and coordinate emergency resources when necessary.
  • Welcome and engage veterans at MVCC; connect with partners and resources.
  • Collect intake information for financial assistance requests and maintain records.

Founded in November 2017, Minority Veterans of America (MVA) is a 501(c)(3) nonprofit organization designed to change the narrative of the American military veteran. MVA builds community around the unique identities that veterans hold - beyond their military service - including veterans of color, women, LGBTQ+ veterans, and religious minorities. By bringing these groups together, MVA creates a diverse and inclusive veteran community focused on belonging, equity, and access.

Role Description

MVA's Case Manager is responsible for providing comprehensive case management services to veterans participating in our Transitional Housing Program (Q'mmunity House) while also supporting veteran services offered through the Minority Vets Community Center (MVCC) financial assistance program. This position combines direct client support, resource navigation, and administrative responsibilities to help veterans achieve housing stability, financial security, and long-term self-sufficiency.

The Case Manager will maintain a small caseload of Transitional Housing residents, allowing for individualized, high-quality support while also assisting veterans accessing services through the MVCC.

Schedule

Full-time, in-person. Monday through Friday, 9:30 AM - 6:00 PM, with occasional evening or weekend hours required for events. General place of duty will consist of the following:

  • Two (2) days per week onsite at the Transitional Housing Program
  • Two (2) days per week onsite at the Minority Vets Community Center
  • One (1) day per week working remotely for documentation, follow-up, care coordination, and administrative responsibilities
Transitional Housing Case Management
  • Provide ongoing case management services for all Transitional Housing residents.
  • Conduct comprehensive assessments to identify each veteran's strengths, barriers, and service needs.
  • Develop, implement, and regularly update individualized housing stability and service plans as outlined in program case management manual and supplementals.
  • Meet with residents regularly to monitor progress toward established goals.
  • Guide residents in attaining permanent housing and maintaining housing stability.
  • Coordinate referrals for healthcare, behavioral health, substance use treatment, employment, education, legal services, and other community resources.
  • Assist residents with benefits applications, including VA benefits, public assistance, and other supportive services.
  • Maintain accurate and timely case notes, assessments, service plans, and program documentation.
  • Monitor participant compliance with program expectations and provide supportive interventions when challenges arise.
  • Identify ways for residents to meaningfully engage as a household.
  • Participate in case conferencing and community partner meetings as needed.
  • Respond appropriately to participant crises and coordinate emergency resources when necessary.
Minority Vets Community Center Services
  • Welcome and engage veterans accessing services at the MVCC.
  • Connect veterans with partner organizations and community resources.
  • Provide resource navigation and follow-up to ensure veterans successfully access requested services.
  • Build and maintain positive relationships with community partners and providers.
  • Collect intake information for clients requesting financial assistance and case management services, provide follow-up and additional resources as needed, and maintain client records with detailed case notes.
  • Maintain data integrity across platforms to ensure accurate reporting.
  • Assist with outreach activities, workshops, and community events as needed.
  • Assist with reporting requirements.
Preferred Qualifications
  • Bachelor's degree in Social Work, Human Services, Psychology, or a related field (or equivalent experience).
  • Experience providing case management to veterans, individuals experiencing homelessness, or other vulnerable populations.
  • Knowledge of VA healthcare, VA benefits, housing programs, and community resources.
  • Strong organizational, documentation, and time management skills.
  • Ability to work independently while collaborating effectively and interdepartmentally with a hybrid and remote team.
  • Excellent interpersonal, communication, and problem-solving skills, as well as personal/professional boundaries.
  • Experience with trauma-informed care, harm reduction, motivational interviewing, and strengths-based case management is preferred.
