Qualifications

  • Bachelor’s degree in social work or related field; or veteran with three years of related experience; or no degree with five years of experience.
  • Demonstrated ability in family needs assessment, access to local resources, advocacy, counseling, and collaboration with service providers; commitment to project principles.
  • Excellent verbal and written communication, strong organization, and creative problem‑solving skills.
  • Valid driver’s license, personal automobile, liability insurance, and willingness to travel up to 70% of the time among served counties.
  • Completion of required case‑management and CTI training within 90 days of hire.
  • Alignment with VOA’s core values: commitment, compassion, diversity, justice, and integrity.

Responsibilities

  • Outreach to increase program awareness and recruit veteran families.
  • Conduct case‑management duties, including assessment, case‑plan development/monitoring, and follow‑up activities.
  • Establish linkages with agencies and service providers.
  • Coordinate with VAMCs and other providers to deliver healthcare and daily living services.
  • Allocate resources based on participant needs.
  • Educate participants on services, rights, and supportive resources.
  • Assist participants in obtaining VA benefits and other public benefits.
  • Provide or refer for supportive services such as financial planning, transportation, income support, fiduciary/representative payee, legal services, childcare, and housing counseling.
  • Complete documentation, progress notes, and HMIS data entry within 48 hours of contact.

Job Summary and Qualifications

The SSVF program assists veterans who are homeless or at‑risk of homelessness in ending their housing crisis. The SSVF Case Manager works to help veterans achieve long‑term housing stability and self‑sufficiency through supportive services, education, and community resource connections.

  • Culturally appropriate interaction with veterans and families.
  • Participate in performance quality improvement duties and PQI committee assignments.
  • Facilitate the CTI evidence‑based model.
  • Collaborate with SSVF Case Managers, Intake Coordinators, and Outreach Workers; attend regular case consultation.
  • Advocate for veterans, integrating cultural values into case plans.
  • Serve as liaison with local HUD‑VASH teams, attend case conferencing, and coordinate locally to meet needs of both urban and rural veterans.
  • Identify systemic barriers; communicate with leadership to develop solutions.
  • Comply with all program policies, procedures, and COC accreditation standards.
