Job summary

Northeast Family Services is seeking a dedicated Case Manager to support families in New Hampshire. The role reports to the Clinical Supervisor and acts as a liaison among the treatment team, clients, families, and DCYF, coordinating in-home services and ensuring high-quality documentation.

You will manage caseloads, develop measurable treatment plans with families, and participate in court-related reporting, providing education and supervision in home and community settings.

Qualifications

  • Bachelor’s degree required in social work, counseling, psychology, or related human services field.
  • Two+ years of experience working with children, youth and families in home/community settings.
  • Valid driver’s license with reliable vehicle access.
  • Authorized to work in the United States (no visa sponsorship).
  • Access to high‑speed, secure internet.

About the Role

The Case Manager will report to the Clinical Supervisor and serve as a liaison for the treatment team, client, families, and collateral service providers.

Responsibilities
  • Maintain assigned caseload and ensure the proper implementation of in-home services in accordance with referrals made by DCYF.
  • Collaborate with DCYF to respond to referral requests, create treatment plans, participate in treatment meetings, and provide regular documentation.
  • Develop and collaborate on treatment plan formation with measurable and attainable goals in partnership with families and DCYF.
  • Adhere to all documentation requirements of service delivery including assessment and report writing.
  • Participate in reporting progress to the court system through in person appearances and written reporting as requested.
  • Provide parent education and supervised visitation to children, adults, and families in home and community-based settings.
  • Offer parent coaching and support including effective parenting strategies, meeting educational needs, understanding developmental milestones, meal preparation and budgeting.
  • Enforce supervised visitation expectations as identified by DCYF to ensure child safety.
  • Interface with DCYF, the NH court system, schools, behavioral health providers, and other involved members of the treatment team as necessary to provide quality service delivery.
  • Successfully assess client risk on an ongoing basis and consult with clinical supervisor regarding any safety concerns.
  • Transport clients, including children and adults, requiring car seat and booster seat installation.
  • Be on call and respond to phone calls to provide support to families as needed.
  • Be available early evening; most visits are in the afternoon or early evening; schedule is subject to client needs.
  • Document all clinical contacts and documentation through the Electronic Health System.
  • Perform other duties as assigned.
Required Experience and Qualifications
  • Bachelor’s Degree in social work, counseling, psychology, or related human services field.
  • Two or more years of experience working with children, youth and families in a home or community setting (preferred).
  • Valid driver’s license with reliable vehicle access.
  • Authorized to work in the United States (no visa sponsorship).
  • Access to high‑speed, secure internet.

Compensation
  • Salary up to 50,000 USD.
  • 500 USD stay‑on bonus after six months.
