Job summary

Northeast Family Services is seeking a Case Manager to coordinate in-home services for children and families in New Hampshire. You will liaise with the treatment team, DCYF, courts, and schools to implement treatment plans and document progress.

The role requires flexible hours, evening availability, and reliable transportation. Responsibilities include developing measurable goals with families, providing parent education, and ensuring safety through ongoing risk assessments.

Qualifications

  • Bachelor’s degree in social work, counseling, psychology or related human services field.
  • Two years minimum experience working with children, youth and families preferred.
  • Valid driver’s license with reliable access to a vehicle.
  • Authorized to work in the U.S. We cannot sponsor visas at this time.
  • Access to high-speed, secure internet is required.

Responsibilities

  • Maintains assigned caseload and ensures proper in-home service delivery per referrals by DCYF.
  • Collaborates with DCYF to respond to referrals, develop treatment plans, and provide documentation.
  • Develops measurable treatment goals with families and DCYF.
  • Portrays thorough documentation including assessments and reports.
  • Reports progress to the court system via appearances and written reports.
  • Provides parent education and supervised visitation in home and community settings.
  • Offers parent coaching on effective parenting strategies and meeting children’s healthcare and educational needs.
  • Ensures child safety and adheres to DCYF supervision expectations.
  • Interfaces with DCYF, NH courts, schools, and other providers to ensure quality service delivery.
  • Assesses client risk regularly and consults with supervisor on safety concerns.
  • Transports clients, including children and adults, with proper car seat installations.
  • Rotates on-call to support families; flexible scheduling in afternoons/early evenings.
  • Documents all clinical contacts in the Electronic Health System.
  • Performs other duties as assigned.

Job description

Case Manager – The Case Manager will report to the Clinical Supervisor. In collaboration with other members of the clinical team, the case manager is responsible for serving as a liaison for the treatment team, client, families and collateral service providers.

Positions are available NOW throughout New Hampshire!

Responsibilities
  • Maintains assigned caseload and ensures the proper implementation of in-home services in accordance with referrals made by DCYF.
  • Works collaboratively with DCYF to respond to referral requests, create treatment plans, participate in treatment meetings, and provide regular service specific documentation.
  • Develops and collaborates on treatment plan formation with measurable and attainable goals in partnership with families and DCYF.
  • Adheres to all documentation requirements of service delivery including assessment writing and report writing.
  • Participates in reporting progress to the court system through in person appearances and written reporting as requested.
  • Provides parent education and supervised visitation to children, adults, and families in home and community-based settings.
  • Offers parent coaching and support including provision of effective parenting strategies, meeting healthcare and educational needs of children, understanding developmental milestones, meal preparation and budgeting.
  • Enforces supervised visitation expectations as identified by DCYF to ensure child safety.
  • Interfaces with DCYF, the NH court system, schools, behavioral health providers, and any other involved members of the treatment team as necessary to provide quality service delivery.
  • Successfully assesses client risk on an ongoing basis and consult with clinical supervisor regarding any safety concerns.
  • Transportation of clients, including children and adults, requiring car seat and booster seat installation.
  • On-call rotation: responsible for responding to phone to provide support to families as utilized.
  • Early evening availability required for working hours. Most visits to clients are in the afternoon or early evenings. Must have flexible availability to work due to this. Schedule/hours are subject to our clients needs.
  • Ability to successfully document all clinical contacts and documentation through Electronic Health System.
  • Other duties as assigned.
Qualifications

  • Valid driver’s license with reliable and consistent access to a vehicle.
