Job summary

Canyon State Academy in Queen Creek, AZ inviting a dedicated Case Manager to supervise and implement case plans aligned with placing agencies and student needs. The role ensures a safe, supportive environment and fosters positive growth for youth on campus.

You will collaborate with the Director of Student Services and program leadership, with regular hours Monday–Friday, 8:00am–5:00pm, and opportunities for career advancement in a campus setting on a 180-acre site.

Qualifications

• BA/BS in a related field.
• Minimum 3 years of case management experience with youth/juvenile placements.
• Must be at least 21 years old.
• Ability to pass criminal background check, drug screen, physical exam, and TB test.

Responsibilities

• As a Case Manager, you will develop and facilitate case plans and treatment programs in coordination with placing agencies and individual student needs.
• Reports to the Director of Student Services, Case Management Coordinator, Unit Manager and/or Program Director depending on location.

What You'll Do

• As a Case Manager, you will be responsible for administrating, developing and facilitating the completion of case plans and treatment programs in accordance with the requests of the placing agency and the needs of each student on their case load. Depending on location, the Case Manager reports to the Director of Student Services, Case Management Coordinator, Unit Manager and/or Program Director.

Minimum Qualifications

• Have a BA/BS in a related field
• have prior case management experience, including 3 years with Youth and/or Juvenile placement agencies
• Must be at least 21 years of age.
• Ability to pass a criminal background check, drug screen, physical examination, and TB test.

Available Shifts

Monday – Friday; 8:00am – 5pm
