Job summary

Canyon State Academy in Queen Creek, AZ is seeking a Behavior Mentor to support youth through individualized life-skills services within a comprehensive behavioral health program.

You will manage a caseload of 25-35 clients, document in an electronic health record system within 24 hours, and collaborate with therapists to promote resilience, recovery, and positive change on a 27.5-hour weekly billing schedule.

Qualifications

• Bachelor’s or associate degree in a behavioral/mental health field.
• Approximately one year of experience in a behavioral health setting.
• Be at least 21 years of age.
• Able to pass criminal background check, drug screen, physical, and TB test.

Responsibilities

• Provide quality case management and individual life-skills services.
• Proficient in learning and utilizing an electronic health record system.
• Complete documentation within 24 hours of service.
• Carry a caseload of 25–35 clients and meet the 27.5-hour weekly billing requirement.
• Coordinate with the assigned therapist and treatment team.
• Promote resilience and recovery with clients using individualized treatment plans.
• Maintain clinical files per licensing requirements.
• Provide crisis intervention as needed and complete incident reports promptly.
