Job summary

Canyon State Academy in Queen Creek, AZ is seeking a Behavior Mentor to support youth through individualized life-skills services within a comprehensive behavioral health program.

You will manage a caseload of 25-35 clients, document in an electronic health record system within 24 hours, and collaborate with therapists to promote resilience, recovery, and positive change on a 27.5-hour weekly billing schedule.

Qualifications

  • Bachelor’s or associate degree in a behavioral/mental health field.
  • Approximately one year of experience in a behavioral health setting.
  • Be at least 21 years of age.
  • Able to pass criminal background check, drug screen, physical, and TB test.

Description

Join Our Dynamic Rite of Passage Team as a Behavior Mentor at Canyon State Academy in Queen Creek, AZ, a community rooted in a rich agricultural history that cultivates growth both in its fields and in the lives of youth by fostering positive change and empowerment.

Canyon State Academy is located on a scenic 180‑acre campus surrounded by a farm, student housing for 300+ youth, a thrift store, café, barbershop and church open to the community incorporating life skills for our students. The campus also features a fully equipped weight room, football stadium, and athletic center (gymnasium, padded wrestling room & more).

What You’ll Do
  • Provide quality case management and individual life‑skills services as indicated by client and program needs.
  • Proficient in learning and utilizing an electronic health record system.
  • Complete documentation in all behavioral health services and/or contacts within 24 hours of the service.
  • Carry a caseload of 25‑35 clients and meet the minimum 27.5‑hour weekly billing requirement.
  • Coordinate with the assigned therapist and/or treatment team.
  • Promote resilience and recovery with clients, utilizing their individualized treatment plans.
  • Maintain clinical files according to agency contract and licensing requirements.
  • Provide crisis intervention as needed and ensure incident reports are completed promptly.
  • Prepare and administer the clinical assessment and treatment plan (if a BHT or higher). Prepare individual progress notes, monthly progress reports, incident reports, meeting or staffing reports, and discharge summaries.
  • Treat others with respect, confront negative behavior, and support constructive confrontations.
  • Complete required reports and documentation in a timely manner, providing management with required reports and advising of any problematic situations.
  • Comply with and implement the Rite of Passage and Rite Path policies and procedures as detailed in the appropriate manuals/handbooks.
  • Maintain the highest standards to prevent illegal, unethical, or improper conduct and ensure the program remains in compliance with agency licensing and Rite of Passage and Rite Path policies and procedures.
  • Assist with the correction of deficiencies and quality improvement efforts.
  • Attend and participate in all required meetings.
  • Commit to attending all training and staff development classes to ensure sufficient training hours annually, notifying the supervisor if training hours are deficient.
  • Complete other duties as assigned, verbally or in written form, to meet program needs.
Qualifications
  • Possess a Bachelor’s (preferred) or associate degree in a related behavioral or mental health field.
  • Approximately one year of experience working in a behavioral health setting.
  • Be at least 21 years of age.
  • Be able to pass a criminal background check, drug screen (no THC testing), physical, and TB test.
  • Be able to pass a search of the child abuse central registry.
