Job summary

Canyon State Academy in Queen Creek, AZ is hiring a Case Manager to administer, develop and facilitate completion of case plans and treatment programs for students. The role reports to the Student Services leadership team and collaborates with placement agencies to meet student needs.

Qualifications include a BA/BS in a related field and at least 3 years of youth/juvenile case management experience, with a minimum age of 21 and ability to pass background checks and health screenings.

As a Case Manager, you will be responsible for administrating, developing and facilitating the completion of case plans and treatment programs in accordance with the requests of the placing agency and the needs of each student on their case load. Minimum Qualifications:

Monday – Friday; 8:00am – 5pm
