Job summary

Canyon State Academy in Queen Creek, AZ is hiring a Case Manager to administer, develop and facilitate completion of case plans and treatment programs for students. The role reports to the Student Services leadership team and collaborates with placement agencies to meet student needs.

Qualifications include a BA/BS in a related field and at least 3 years of youth/juvenile case management experience, with a minimum age of 21 and ability to pass background checks and health screenings.

Qualifications

• Have a BA/BS in a related field.
• 3 years with Youth and/or Juvenile placement agencies.
• Must be at least 21 years of age.
• Ability to pass a criminal background check, drug screen, physical examination, and TB test.
• Schedule subject to change based on the need of the program.

Responsibilities

• Administrating, developing and facilitating the completion of case plans and treatment programs in accordance with the requests of the placing agency and the needs of each student on their case load.
• Depending on location, the Case Manager reports to the Director of Student Services, Case Management Coordinator, Unit Manager and/or Program Director.
