Job summary

Volunteers of America-Greater New York is seeking a Case Manager/Client Care Coordinator to assess client needs, plan services, and link them to resources within the VOA case management model.

The role emphasizes intake assessment, client counseling, and advocacy, with potential supervision of direct care staff. Domestic violence experience is preferred, and a strong commitment to serving underserved populations is essential.

Qualifications

• Minimum qualifications: Associate's degree or 60 college credits in Human Services, or a High School Diploma/GED with 2 years of related experience.
• Demonstrated ability in verbal and written communication, including basic computer skills.
• Domestic violence experience preferred.

Responsibilities

• Manage client intake and assessment to orient clients to the program.
• Develop mutually agreed-upon service plans with clients and update as needed.
• Meet regularly with clients to help obtain needed services/benefits and provide supportive counseling.
• Document case management activities per agency/funder guidelines and collect data for evaluation.
• Provide aftercare to clients who are discharged from the facility.

We are currently seeking qualified candidates for the following position:

Position: Case Manager/Client Care Coordinator

The Case Manager/Client Care Coordinator is a key provider of services within Volunteers of America’s Case Management Model. The Case Manager works with the client to assess their strengths and needs, plan a course of action, link the client to services, counsel the client and monitor progress. S/he must assure that clients receive appropriate, competent and professional information and advice, referrals and advocacy services to achieve successful outcomes. May include supervision of direct care staff.

Minimum Qualifications

Case Manager/Client Care Coordinator Principal Responsibilities

• Manage the client intake and assessment process to orient the client to the program and produce a comprehensive assessment of client service needs.
• Develop with the client a mutually agreed-upon individual service plan reflecting assessment findings goals. Update the plan with the client as required.
• Meet regularly with the client to assist him/her in obtaining needed services/benefits to achieve their individual service plan goals. Provide supportive counseling to strengthen his/her ability to make appropriate life decisions.
• Document case management activities in accordance with agency and funder guidelines and procedures. Collect service data and participate in evaluation and quality assurance activities.
• Provide aftercare to assigned clients who have been placed/discharged from the facility.
