Northeast Counseling Services (NCS) is seeking a Blended Case Manager to provide community-based support to individuals with mental illness. You will work where clients live and participate in their communities, with a flexible schedule to promote work-life balance.

The role uses a strengths-based, recovery-oriented approach to help clients access resources, build independence, and advocate for their needs. A generous benefits package is provided.

Qualifications

• High School Diploma or GED required.
• 12 college credits in sociology, human services, psychology or related field.
• Minimum 2 years of experience in human services with 1 year of direct client contact.
• Bachelor's degree in related field preferred.
• Valid Driver's License required.
• Successful FBI, PA Criminal, and Child Abuse clearances.

Responsibilities

• Provide blended case management in community settings.
• Empower clients to identify needs and access resources.
• Coordinate services and advocate for greater independence.

The Blended Case Manager works with individuals experiencing serious mental illness using a strengths-based, recovery-oriented approach. The primary goal of this position is to empower clients to identify their needs, access resources, and achieve greater independence through coordinated support and advocacy.

Job Type: Full-time, flexible hours

Pay: up to $37,000.00 per year based on education and experience and bilingual ability

Education:
