Job summary

Northeast Counseling Services (NCS) is seeking a Blended Case Manager to support individuals with serious mental illness in their homes and communities. This role emphasizes a strengths-based, recovery-oriented approach and coordinated advocacy to promote independence.

You will work in community settings with a flexible schedule, require a PA driver's license, and have opportunities through a generous benefits package including health insurance, 401k, paid leave, and holidays.

Qualifications

• High School Diploma or GED is required.
• Minimum of 12 college credit hours in Sociology, Human Services, Psychology, or another social science field is required.
• Bachelor's Degree in Social Work, Psychology, Human Services, Sociology, or a related field is preferred.

Responsibilities

• Provide community-based case management to individuals with serious mental illness.
• Utilize a strengths-based, recovery-oriented approach to empower clients.
• Coordinate resources and advocacy to help clients access services and achieve independence.

Education
