Job summary

Northeast Counseling Services is seeking a Blended Case Manager to support individuals with serious mental illness in community settings, helping them access resources, set goals, and increase independence.

The role emphasizes strengths-based, person-centered care, hands-on advocacy, and coordination across services. A flexible schedule and meaningful, field-based work are offered with a comprehensive benefits package. This position will involve driving to client sites as needed.

Qualifications

• High School Diploma or GED is required.
• Minimum of 12 college credit hours in Sociology, Human Services, Psychology, or another social science field is required.
• Two years of experience in Human Services with 1 year of providing direct client contact.
• Bachelor''s Degree in Social Work, Psychology, Human Services, Sociology, or a related field preferred.

Responsibilities

• Empower clients to identify needs and access resources.
• Coordinate support across services and advocate for clients.
• Work in community settings—home, work, and community activities.
• Maintain accurate documentation and collaborate with team members.

Education

Bachelor''s Degree in Social Work, Psychology, Human Services, Sociology, or a related field

Northeast Counseling Services (NCS) is seeking a compassionate, motivated, and dedicated individual to join our team as a Blended Case Manager. The Blended Case Manager works with individuals experiencing serious mental illness using a strengths-based, recovery-oriented approach. The primary goal of this position is to empower clients to identify their needs, access resources, and achieve greater independence through coordinated support and advocacy.

Job Type: Full-time, flexible hours

Pay: $31,200.00 per year to $33,000.00 per year and up based on education and experience
