Job summary

Northeast Counseling Services (NCS) is seeking a Blended Case Manager to support individuals with serious mental illness in their communities, helping them access resources and increase independence. Schedule is Monday–Friday with a flexible option; valid PA driver’s license and high school diploma required.

Qualifications

• High school diploma or GED required.
• Minimum of 12 college credit hours in Sociology, Human Services, Psychology, or another social science field.
• Two(2) years of experience in Human Services with 1 year of providing direct client contact.
• Bachelor's Degree in Social Work, Psychology, Human Services, Sociology, or a related field preferred.

Responsibilities

• Provide blended case management in community settings.
• Empower clients to identify needs and access resources.
• Advocate to promote independence and better quality of life.
