Job summary

Community Care is seeking a Case Manager to join our dedicated team in Dane Co, Wisconsin. The role emphasizes collaboration with an RN Case Manager, complete person-centered assessments, and coordinated care planning to support elderly and disabled adults living independently in the community.

The successful candidate will hold a Bachelor’s in Human Services or related field, have experience with seniors or disabilities, and be eligible for long-term care screening certification.

Qualifications

• Bachelor’s degree in Human Services or closely related field is required.
• One year of experience with older adults or persons with disabilities in a social service capacity is preferred.
• Eligibility for long-term care functional screener certification is required.
• Regular local travel with a valid driver’s license and insurance is expected.

Responsibilities

This full-time opportunity offers a weekday schedule and a $8,000 SIGN-ON BONUS. Position located in Dane Co.

Case Manager Job Responsibilities:

• Manages an assigned caseload of program members in collaboration with an RN Case Manager
• Completes comprehensive member assessments, member centered care plans, & functional screens
• Provides ongoing care coordination and quality assurance monitoring of services
• Maintains required documentation in adherence to established standards.

Case Manager Job Requirements:

• Education: Bachelor’s Degree in a Human Services or closely related area required. A minimum of three (3) years’ experience working with seniors or adults with physical and/or intellectual disabilities may be substituted for a non-human services degree.
• Experience: One year of experience with older adults or persons with physical and or intellectual disabilities in a social service capacity preferred.
• Certification: Eligibility for long-term care functional screener certification is required
• Travel: Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.

Case Manager Job Schedule:

• Monday to Friday, 8:00 AM-4:30 PM
