Community Care, Inc. is hiring a Case Manager/Care Manager to join our Family Care Program team. This role supports community-based case management for older adults and adults with disabilities, coordinating care and ensuring services are delivered with dignity and respect.

The position emphasizes teamwork, flexible telework options, and commitment to our mission-driven community. A bachelor's degree in human services and relevant experience are required.

• Bachelor's Degree in a Human Services Area required.
• One year of experience with older adults or persons with physical and/or intellectual disabilities in a social service capacity required.
• Eligibility for long-term care functional screener certification is required.

This is a Pool Position It is based in Milwaukee County with telework currently being the norm.

Case Manager/Care Manager Job Responsibilities:

• Manages an assigned caseload of program members in collaboration with an RN Care Manager
• Completes comprehensive member assessments, member centered care plans, & functional screens
• Provides ongoing care coordination and quality assurance monitoring of services
• Maintains required documentation in adherence to established standards.

Case Manager/Care Manager Job Requirements:

• Education: Bachelor's Degree in a Human Services Area required.
• Experience: One year of experience with older adults or persons with physical and or intellectual disabilities in a social service capacity required.
• Certification: Eligibility for long-term care functional screener certification is requiredTravel : Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.

Care Manager Job Schedule:

• Monday to Friday
• Generally during business hours of operation (8AM-4:30PM)
