Job summary

Community Care in Milwaukee, WI is hiring a Case Manager/Care Manager Day Shift for our Family Care Program. This full-time role runs Monday–Friday, 8:00 a.m.–4:30 p.m., with an $8,000 sign-on bonus and a focus on helping elderly individuals and adults with disabilities live independently.

You will manage caseloads, complete comprehensive assessments, develop member-centered care plans, coordinate care, educate members, and monitor services. Local travel is required using your own vehicle.

Community Care is hiring a Case Manager/Care Manager Day Shift for our Family Care Program in Milwaukee, WI. Offering an $8,000 sign-on bonus! This is a Monday through Friday position, 8:00 a.m. to 4:30 p.m.

Case Manager/Care Manager Job Responsibilities:

Case Manager/Care Manager Job Requirements:

• Education: Bachelor's Degree in Human Services or closely related field required.
• Licensure: Current long-term care functional screener certification preferred (eligibility for this certification is required)
• Experience: Minimum of 3 years recent experience with older adults or persons with physical or intellectual disabilities is required. 3 years of work in community nursing preferred.
