HSHS Rehabilitation Hospital in Green Bay, WI, is seeking a Case Manager to coordinate patient care across disciplines, develop IPoC, and ensure CMS compliance. This full-time role emphasizes efficient resource use, high-quality service, and cost-effective outcomes during inpatient rehabilitation.
You will participate in weekly care team conferences, document per workflow timelines, and support discharge planning while communicating with patients, families and care teams.

The Case Manager (CM) Coordinates patient care to ensure efficient resource use, high-quality service delivery, and compliance with regulatory standards. Conducts assessments, develops and implements care plans, and evaluates patient needs to support continuity of care and cost-effective outcomes. Collaborates with interdisciplinary teams, communicates with patients and families, and advocates for patient needs through care coordination and discharge planning. Upholds professional standards while supporting the organization's mission, vision, and values.
