Job summary

The Case Manager role at HSHS Rehabilitation Hospital, part of Lifepoint Health, coordinates patient care for efficient resource use and high-quality service in a 40-bed inpatient rehab setting in Green Bay, WI. You will work with interdisciplinary teams to assess needs, develop care plans, and support discharge planning.

Applicants should hold a valid nursing or social work license; CM/rehab nursing certification is preferred.

Qualifications

• Current Registered Nurse or Social Work licensure, or other healthcare professional licensure.
• Case Management or Rehabilitation Nursing certification preferred (CCM; ARN/ACM; RN-BC).
• Minimum of 2 years social work or case management experience in an inpatient setting; rehab experience preferred.
• Effective oral and written communication skills in English; additional languages a plus.
• Basic computer skills in Excel, Word, Outlook, PowerPoint; able to interpret data and document accurately.

Responsibilities

• Coordinate patient care to ensure efficient resource use and compliant service delivery.
• Conduct assessments, develop and implement IPoC per CMS guidelines and safety standards.
• Schedule family conferences and inform caregivers about progress and discharge plans.
• Coordinate weekly care team conferences to refine treatment plans and goals.
• Review CMG and liaise with HIM to address potential comorbid issues during stay.
• Communicate with nursing, therapy and other departments to ensure proper utilization.

How You'll Contribute

The Case Manager (CM) Coordinates patient care to ensure efficient resource use, high-quality service delivery, and compliance with regulatory standards. Conducts assessments, develops and implements care plans, and evaluates patient needs to support continuity of care and cost-effective outcomes. Collaborates with interdisciplinary teams, communicates with patients and families, and advocates for patient needs through care coordination and discharge planning. Upholds professional standards while supporting the organization's mission, vision, and values.

• As appropriate, consults other departmental staff to collaborate in patient care delivery, identify barriers to care and or discharge and develop solutions/resolution.
• Completes documentation per workflow timeline and content requirements including completion of the Individual Plan of Care (IPoC) per CMS guidelines.
• Schedules family conferences and/or communicates with caregiver following each team conference and more often as needed to keep patient and designated caregiver informed of progress and provides appropriate information related to goal achievement, course of rehabilitation stay, and plans for discharge.
• Coordinates weekly patient care team conferences to facilitate development, monitoring and refinement of treatment plan to achieve identified patient goals and outcomes.
• Reviews the patient's assigned CMG and helps the team identify any potential missed comorbid conditions that are actively being treated during the patient's stay. Communicate any findings to the HIM team.
• Communicates effectively with nursing, therapy and other ancillary departments to ensure proper utilization.

What We're Looking For

Applicants should possess the following qualifications and skills:

• Current Registered Nurse or Social Work licensure or Healthcare professional licensure as Respiratory Therapist, Physical Therapist, Speech Language Pathologist or Occupational Therapist.
• Certification in Case Management or Rehabilitation Nursing preferred; for example, Commission for Case Manager Certification (CCM); Association of Rehabilitation Nurses (ARN) certification, American Case Management Association (ACM) or Board Certification in CM by the ANCC e.g.: RN-BC
• Minimum of 2 years social work or case management experience in an inpatient setting highly preferred; acute/rehabilitation hospital experience preferred.
• Effective oral and written communication skills in English, additional languages preferred.
• Basic computer skills in excel, word, outlook, power point, etc. required.
• Must have good organizational skills, time management skills and analytical ability in order to interpret information and carry out duties independently.
