Job summary

PeaceHealth Springfield Rehabilitation Hospital, part of Lifepoint Health, seeks a Case Manager for a PRN role in Oregon. You will participate in pre-admission screening, conduct assessments, and develop individualized plans to support patient recovery.

Your responsibilities include documenting care, maintaining therapeutic relationships with patients and families, and providing education and resources for continued independence.

Qualifications

  • Current RN, LCSW, RT, PT, SLP licensure in OR.
  • Certification in Case Management or Rehabilitation Nursing preferred; examples CCM/ARN/ACM/RN-BC.
  • Minimum of 2 years social work or case management experience in an inpatient setting; acute/rehabilitation experience preferred.
  • Effective oral and written communication skills in English; additional languages preferred.
  • Basic computer skills in Excel, Word, Outlook, PowerPoint.
  • Good organizational skills, time management, and analytical ability to interpret information.
  • Must be cooperative and a team player; maintain confidentiality.

Responsibilities

  • Participate in pre-admission screening as requested.
  • Conduct initial assessments.
  • Develop individualized treatment plans.
  • Provide age-appropriate therapeutic interventions.
  • Evaluate patient and family response to treatment.
  • Document the course of patient care including progress and ongoing need for treatment.
  • Establishes and maintains a therapeutic relationship with patients and families.
  • Provide patient and family education on tasks and resources for continued functional independence.
  • Obtain pre-certification and re-certification of insurance coverage as appropriate per policy.
  • Participate in continuing education/professional development.
  • Communicate and demonstrate a professional image/attitude for patients, families, coworkers and others.
  • Contribute to client relations and marketing activities.
  • Maintain competency, licenses/certifications, and memberships in applicable professional organizations.
  • Other duties as assigned.

How You'll Contribute

A Case Manager who excels in this role:

What We're Looking For
  • Current RN, LCSW, RT, PT, SLP licensure in the state of OR.
  • Certification in Case Management or Rehabilitation Nursing preferred; for example, Commission for Case Manager Certification (CCM); Association of Rehabilitation Nurses (ARN) certification, American Case Management Association (ACM) or Board Certification in CM by the ANCC e.g.: RN-BC
  • Minimum of 2 years social work or case management experience in an inpatient setting highly preferred; acute/rehabilitation hospital experience preferred.
  • Effective oral and written communication skills in English, additional languages preferred.
  • Basic computer skills in excel, word, outlook, power point, etc. required.
  • Must have good organizational skills, time management skills and analytical ability in order to interpret information and carry out duties independently
  • Must be cooperative and have the desire to be a team player.
