Job summary

Solero Youngstown is seeking a dedicated case manager to support adults in a structured outpatient setting in Youngstown, Ohio. The role focuses on assessment, service planning, and coordination with clinical staff to promote recovery and access to housing, employment, and healthcare resources.

The ideal candidate holds an Ohio CDCA certification, a high school diploma or GED, and is 21 years or older. Newly qualified grads are welcome to apply in a thriving behavioral health program.

Responsibilities

• Provide case management services, including assessment, service planning, and ongoing support.
• Utilize peer support principles to engage and motivate clients in recovery.
• Assist clients in accessing community resources such as housing, employment, and healthcare.
• Maintain accurate and timely documentation in compliance with agency and state guidelines.
• Collaborate with clinical staff to coordinate care and support treatment goals.
• Facilitate individual and group sessions as appropriate.
• Advocate for clients and promote a recovery-oriented environment.

About Solero Behavioral Transitions

Solero Behavioral Transitions in offers a safe, structured environment for adults managing substance use and mental health challenges. As a dual diagnosis outpatient program, we provide integrated treatment for individuals with primary substance use or mental health disorders.

Key Responsibilities

• Provide case management services, including assessment, service planning, and ongoing support
