The Greater Community AIDS Project (GCAP) is seeking a compassionate, motivated Case Manager to support clients transitioning into permanent supportive housing while connecting them to essential healthcare, employment, and community resources.

What You’ll Do:

• Motivational interviewing and strengths-based case management experience
• Trauma-informed care approach
• Strong communication, organization, and documentation skills
• Valid driver’s license, reliable transportation, clean driving record, and willingness to transport clients
• Bachelor’s degree in social work, Human Services, Psychology, Sociology, or related field preferred OR Bachelor’s degree in another field with at least 20 semester hours in human services/social services
