Job summary

Housing Works Inc. in New York is seeking a Housing Case Manager to provide comprehensive case management and care coordination for residents living with HIV and co-occurring conditions.

You’ll work in clients’ homes, in the office, and in the community to promote housing stability and independent living skills. Requirements include a bachelor’s degree in a related field and at least three years of social services experience with homelessness, mental health, or substance use.

Qualifications

• Bachelor's degree in social work, psychology, counseling, or a related field preferred.
• A minimum of three years of relevant experience in a social services setting working with individuals who have experienced homelessness, mental health issues, and/or substance use is required

Responsibilities

• HCM provides case management services to assigned clients, including supportive counseling, motivational interviewing, crisis intervention, care coordination, linkage and referral, assessment and service plan development, field work, and assessment/observation of resident in their living space as needed but no less than once per month.
• Documentation of all services and interventions provided as per funder, regulatory, industry and internal standards.

Job description

• Pay or shift range: $24 USD to $31.25 USD
  The estimated range is the budgeted amount for this position. Final offers are based on various factors, including skill set, experience, location, qualifications and other job-related reasons.

Overview:

The Housing Case Manager (HCM) provides comprehensive Case Management services and care coordination to a caseload of residents who are living with HIV and/or and co-occurring conditions. The Housing Case Manager works closely with their clients in the clients’ homes, in the office, and in the community helping to ensure housing stability by promoting independent living skills, financial management, and health and wellness.
