Job summary

Union County Community Action, Inc. is seeking a compassionate Case Manager to connect families with resources, support children’s health and school readiness, and help families achieve their goals through Head Start services.

The role requires a Bachelor’s degree in a related field, 1–3 years of experience, and a valid driver’s license. The starting rate is $19.58 per hour, with a Monday–Friday schedule and 40 hours per week.

Qualifications

• Bachelor's degree required or in progress in a related field.
• 1–3 years of related experience preferred in case management, family services, or community programs.
• CDL required within 90 days of employment and valid driver's license.

Responsibilities

• Recruit and enroll eligible children and families into Head Start.
• Develop and maintain Family Partnership Agreements with enrolled families.
• Connect families with community resources and support services.
• Assist families in scheduling and accessing health and developmental services.
• Conduct home visits and provide ongoing family support.
• Support parent engagement activities, meetings and workshops.
• Maintain accurate records and data entry within ChildPlus.
• Collaborate with site staff and community partners to coordinate services.
• Assist with program recruitment and outreach efforts.
• Support transportation services as assigned.

Physical Requirements

Must be able to travel within the service area, conduct home visits, attend community events, move throughout program sites, and occasionally lift or move up to 50 pounds.
