We are seeking a highly motivated and experienced Case Manager to join our dynamic team. Our ideal candidate will have a passion for social work, with a focus on dealing with homeless and mentally ill individuals, and a deep understanding of the nonprofit sector. This role requires a minimum of 2 years of experience in a similar position. You will be responsible for coordinating and providing case management services, including creating treatment plans and coordinating care with other professionals. This is an excellent opportunity to make a significant impact on people's lives while working in a fast-paced, rewarding environment.

Responsibilities
  • Develop, implement, and review individual case management plans for clients, including assessment of needs, setting goals, and coordinating services.
  • Coordinate with various stakeholders such as social workers, psychologists, and other healthcare professionals to ensure comprehensive care.
  • Advocate for clients' needs and rights, and assist with crisis intervention as needed.
  • Maintain accurate and up-to-date client records, and ensure compliance with all relevant laws and regulations.
  • Provide emotional support and counseling to clients, and refer them to appropriate resources as needed.
  • Monitor and evaluate clients' progress, and adjust case management plans as necessary.
  • Conduct regular meetings with clients to review their progress and address any issues or concerns.
  • Collaborate with other team members to ensure a comprehensive, integrated approach to client care.
  • Participate in ongoing professional development and training opportunities to stay current with best practices in case management and social work.
Qualifications
  • Bachelor's degree in Social Work, Psychology, Counseling, or a related field.
  • A minimum of 2 years of experience in case management, preferably in the nonprofit sector.
  • Previous experience working with homeless and mentally ill individuals.
  • Excellent interpersonal and communication skills, with the ability to build strong relationships with clients and other stakeholders.
  • Strong organizational and case management skills, with the ability to manage multiple cases simultaneously.
  • Knowledge of relevant laws and regulations related to case management.
  • Ability to work effectively in a team-oriented environment.
  • Strong problem-solving skills, with the ability to think critically and make informed decisions.
  • Ability to handle sensitive information with discretion and maintain client confidentiality.
  • Proficiency in Microsoft Office Suite and case management software.
  • Valid driver’s license and reliable transportation.
