Chimes-Holcomb is seeking a Case Manager in Allentown, PA to support behavioral health services. You will manage up to 30 clients, develop recovery-focused service plans, and connect individuals with housing, healthcare, employment, and community resources.

Responsibilities include monitoring progress, crisis intervention, collaboration with families and providers, and maintaining documentation. A rotating 24/7 on-call schedule is required.

Pay Rate: $20 /hour

Schedule: Full-Time | Monday-Friday, 8:30 AM-4:30 PM

In this community-based role, you'll help individuals receiving behavioral health services access essential resources, overcome challenges, and work toward their personal recovery goals.

What You'll Do

• Manage a caseload of up to 30 individuals receiving behavioral health services.
• Develop personalized service plans that promote recovery and independence.
• Connect clients with housing, healthcare, employment, and community resources.

What We're Looking For

• Bachelor's degree in Psychology, Social Work, Criminal Justice, Counseling, Education, or a related field OR
• High school diploma with qualifying social science credits and relevant human services experience.
• Valid Pennsylvania driver's license with no more than 3 points.
• Strong communication, organizational, and problem-solving skills.
• Ability to complete required background clearances.

Additional education or experience may be considered in accordance with program requirements.
