Chimes-Holcomb is seeking a Case Manager in Allentown, PA to support behavioral health services. You will manage up to 30 clients, develop recovery-focused service plans, and connect individuals with housing, healthcare, employment, and community resources.

Responsibilities include monitoring progress, crisis intervention, collaboration with families and providers, and maintaining documentation. A rotating 24/7 on-call schedule is required.

Qualifications

• Bachelor's degree or high school with relevant experience required.
• Strong communication, organizational, and problem-solving skills essential.
• Ability to complete required background clearances and maintain accurate documentation.

Responsibilities

• Manage a caseload of up to 30 individuals receiving behavioral health services.
• Develop personalized service plans to promote recovery and independence.
• Connect clients with housing, healthcare, employment, and community resources.
• Monitor client progress and provide crisis intervention when needed.
• Collaborate with families, providers, and community support teams.
• Maintain accurate and timely documentation.
• Participate in a rotating 24/7 on-call schedule.
