Chimes is seeking a Case Manager to provide community-based behavioral health services and assess resident resources, helping clients live safely in the community.
Responsibilities include managing up to 30 clients, developing strength-based plans, coordinating housing and healthcare resources, and providing crisis intervention as needed.

The Case Manager will provide community-based services to individuals identified as eligible for behavioral health services, including assessing and monitoring resources that help the client live in the community in a stable and safe manner
