Chimes-Holcomb is seeking a Case Manager for Behavioral Health Services in Allentown, PA. This community-based role supports individuals with behavioral health services to access essential resources and work toward personal recovery goals.

Responsibilities include managing a caseload, developing recovery-focused plans, and coordinating housing, healthcare, employment, and community supports. On-call duties rotate as part of the schedule.

Pay Rate: $20 /hour

Schedule: Full-Time | Monday-Friday, 8:30 AM-4:30 PM

What You'll Do

• Manage a caseload of up to 30 individuals receiving behavioral health services.
• Develop personalized service plans that promote recovery and independence.
• Connect clients with housing, healthcare, employment, and community resources.
• Monitor client progress and provide crisis intervention when needed.
• Collaborate with families, providers, and community support teams.
• Maintain accurate and timely documentation.
• Participate in a rotating 24/7 on-call schedule.

What We're Looking For

• Bachelor's degree in Psychology, Social Work, Criminal Justice, Counseling, Education, or a related field OR
• High school diploma with qualifying social science credits and relevant human services experience.
• Valid Pennsylvania driver's license with no more than 3 points.
• Strong communication, organizational, and problem-solving skills.
• Ability to complete required background clearances.

Additional education or experience may be considered in accordance with program requirements.
