Job summary

Chimes-Holcomb is seeking a Case Manager in Allentown, PA to support individuals with behavioral health services. The role involves managing a caseload, creating recovery-focused service plans, and linking clients to housing, healthcare, employment, and community supports.

The position requires a Bachelor’s degree in a related field or a high school diploma with relevant human services experience, plus a PA driver's license and clearances. Competitive pay and comprehensive benefits are offered.

Qualifications

• Bachelor's degree in Psychology, Social Work, Criminal Justice, Counseling, Education, or related field or high school diploma with qualifying social science credits and relevant human services experience.
• Valid Pennsylvania driver’s license with no more than 3 points.
• Strong communication, organizational, and problem-solving skills.
• Ability to complete required background clearances.

Responsibilities

• Manage a caseload of up to 30 individuals receiving behavioral health services.
• Develop personalized service plans that promote recovery and independence.
• Connect clients with housing, healthcare, employment, and community resources.
• Monitor client progress and provide crisis intervention when needed.
• Collaborate with families, providers, and community support teams.
• Maintain accurate and timely documentation.
• Participate in a rotating 24/7 on-call schedule.

What We're Looking For
