Job summary

Chimes-Holcomb is seeking a Case Manager in Allentown, PA to help behavioral health clients access essential resources and work toward recovery goals. The role requires a Bachelor’s degree in a related field or a suitable high school diploma with relevant experience.

You will manage a caseload of up to 30 individuals, develop personalized plans, link clients with housing, healthcare, employment, and community resources, monitor progress, and participate in a rotating 24/7 on-call schedule.

Qualifications

• Bachelor's degree in Psychology, Social Work, Criminal Justice, Counseling, Education, or a related field; OR
• High school diploma with qualifying social science credits and relevant human services experience.
• Valid Pennsylvania driver's license with no more than 3 points.
• Ability to complete required background clearances.
• Additional education or experience may be considered per program requirements.

Responsibilities
