Job summary

HonorHealth is seeking a Case Management Assistant for our Deer Valley location in Phoenix, AZ. In this in-person role, you will assist directors and case managers with workflow, information tracking, and resource coordination to optimize care management processes.

The position emphasizes clerical duties, patient communication, and collaboration with insurance and community resources to ensure timely post-discharge services.

Qualifications

• High School Diploma or GED required.
• Associates degree preferred.
• 1 year clerical support experience required.
• 1 year administrative support in a clinical environment or with medical terminology/case management preferred.

Responsibilities

• Performs routine clerical tasks and supports case managers.
• Compiles information, organizes files, and coordinates with outside agencies.
• Monitors case status and coordinates with patients and service units.
• Obtains forms for placement and initiates post-discharge processes.
• Arranges transportation and gathers info from insurance for post-discharge coverage.
• Prioritizes placement requests and coordinates with the Case Management team.
