Job summary

HonorHealth is seeking an RN Case Manager for our Deer Valley location in Phoenix, AZ. This full-time role features day shifts, excellent sign-on incentives up to $10,000, and a collaborative team focused on safe, high-quality discharge planning.

The position requires an RN license, 3 years of clinical hospital experience, and 5 years in case management preferred. The role emphasizes care coordination, utilization review, and patient advocacy within a structured hospital setting.

Qualifications

• Associates in Nursing is required; BSN preferred.
• RN licensure in good standing to practice in Arizona.
• Experience in hospital case management settings is highly preferred.

Responsibilities

• Guide care coordination for high risk acute care patients and assist in discharge planning.
• Analyze medical records to determine admission necessity and length of stay per guidelines.
• Advocate for patients and coordinate with the care team to meet health needs.
• Collaborate with physicians and resources to ensure timely, quality services.
• Support transitions of care and maintain required logs and reports.
