Pinnacle Services in Minneapolis is seeking a Case Management Supervisor to lead a team of Case Manager Captains, and to manage our health plans and Dakota County contract. If you have experience with the Minnesota waivers and Health Care Programs, this is a great opportunity to advance your career with Pinnacle Services.

Qualifications

• Graduate with a four-year degree in social work, psychology, sociology or related field.
• 3–5 years supervisory experience in social services, preferably case management.
• 2+ years' experience with waivered Case Management (CADI, DD, AC/EW).
• Valid driver's license, satisfactory driving record, and vehicle with current insurance.
• Office-based role with remote flexibility 1 day/week after 90 days.

Responsibilities

• Attend County meetings and support case management operations.
• Train new Case Managers, Client Relations Coordinators and Case Manager Captains.
• Provide supervision and support to Case Manager Captains.
• Ensure department goals, contract and licensing compliance.
• Contribute to business development and policy updates.
