• Official transcript required to verify coursework in developmental disabilities (where applicable).

Responsibilities

• Assess areas where the person served needs assistance.
• Coordinate resources for the person served.
• Monitor service delivery and work with providers to meet needs.
• Develop and implement service plans.
• Attend regular meetings with person served and interdisciplinary teams.
• Record case notes for billable units.
