Qualifications

• Graduate from an accredited four-year college with a major in social work, psychology, sociology, or a closely related field.
• Or graduate from an accredited four-year college with a major in any field.
• One year of full-time experience as a social worker/case manager/care coordinator in a public or private social service agency.
• Valid driver’s license and vehicle with current insurance.
• Must clear local/state background checks.

Responsibilities

• Assess areas of the person served life where assistance is needed.
• Offer and coordinate resources for the person served being supported.
• Monitor service delivery and work with providers to ensure the person’s needs are met.
• Develop service plans
• Attend regular meetings with the person served and their interdisciplinary teams.
• Document case notes to create billable units.
