Pinnacle Services is seeking a motivated Case Manager to join our Dakota County team. The role emphasizes independent caseload management, collaborative consulting, and delivering high‑quality services to residents across the six‑county metro area.

The position requires a bachelor’s degree in social work or related field, with licensure being encouraged. We offer a supportive, growth‑focused environment, a hybrid schedule after training, and a comprehensive benefits package.

Qualifications

• Be a graduate from an accredited four‑year college with a major in social work, psychology, sociology, or a closely related field.
• Be a graduate from an accredited four‑year college with a major in any field.
• One year of full‑time experience as a social worker/case manager/care coordinator in a public or private social service agency.
• One year of experience in education or treatment of persons served with developmental disabilities or related conditions.
• At least one course focused on developmental disabilities is required. Transcript verification.
• Valid driver’s license and acceptable driving record per Pinnacle Services policy; insured vehicle.
• Successful background checks (local and/or state).

Responsibilities

• Assess the areas of the persons served life where assistance is needed.
• Offer and coordinate resources for the persons served being supported.
• Monitor service delivery and work with providers to ensure the individual's needs are met.
• Develop service plans.
• Attend regular in‑person meetings with persons served and their interdisciplinary teams.
• Maintain and create accurate Case Notes and Billable Units.
• Stay up to date with case management systems, software, and DHS and County resources.

Required Qualifications

• Be a graduate from an accredited four‑year college with a major in social work, psychology, sociology, or a closely related field
• Be a graduate from an accredited four‑year college with a major in any field
• One year of full‑time experience as a social worker/case manager/care coordinator in a public or private social service agency
• One year of experience in education or treatment of persons served with developmental disabilities or related conditions
• At least one course focused on developmental disabilities is required. Candidates must provide an official college or university transcript to verify completion of this coursework.
• Applicants must have a valid driver’s license and acceptable driving record as determined by Pinnacle Services policy. Applicants must have a vehicle that is covered with current insurance.
• Applicants must successfully clear local and/or state background checks

Preferred Qualifications

• Possess a LSW or higher form of BOSW licensure
• DD Waiver Specific – Bilingual in Spanish
