The role focuses on stabilization, independence, and skills development within a supportive environment, coordinating with external providers. The supervisor will ensure plans are current, compliant, and tailored to resident needs, and will supervise staff with guidance, training, and performance reviews.

Qualifications

• Bachelor’s degree in social work or psychology required with five years of experience plus two years of supervisory experience in social services.
• Professional verbal and written communication skills; proficiency in English; multilingual and Spanish prioritized.

Responsibilities

• Coordinate and supervise service and care plans for residents to promote stability and independence.
• Lead and support Case Managers; ensure plans are updated and aligned with biopsychosocial assessments.
• Develop partnerships with external providers and community-based services to support residents.
