Job summary

ACMH in the Bronx is seeking a Senior Case Manager/Assistant Program Director for the Bronx Supportive Apartment program. You will coordinate case management services, conduct home visits, and support the Program Director in daily operations to ensure high-quality community rehabilitation services.

The role requires a master’s degree or a related field and experience in mental health residential settings, with the ability to lead and mentor others.

Qualifications

• Master’s degree in psychology or related field or Bachelor’s with significant experience.
• Understanding of evidence-based practices in rehabilitation and recovery.
• Experience in mental health residential settings.
• Ability to lead and mentor others.

Responsibilities

• Conducts home visits on a regular basis for assigned case load.
• Obtains psychosocial histories and conducts functional skills assessments.
• Establishes and maintains liaison with primary provider of psychiatric care.
• Assures access to psychiatric, substance abuse and medical services, and entitlement benefits.
• Develops an individualized rehabilitation plan in accordance with client needs and goals.
• Delivers community rehabilitation services according to individual need.
• Documents client response to service and updates the Service/Support Plan.
• Prepares residents for transition to independent living and makes referrals.
• Facilitates crisis intervention in emergencies.
• Oversees maintenance of apartments and reports issues to supervisor.
• Serves as Qualified Mental Health Staff Person and ensures policy compliance.

Education

Master’s degree in psychology or related field
