Job summary

Matthew 25 Aids Services in Kentucky seeks a dedicated case manager to provide medical case management under the KHCCP, assess social needs, and connect HIV+ individuals with community resources. This role supports housing initiatives and coordinates with local programs to ensure comprehensive client care.

You’ll collaborate with the Housing Coordinator, promote Housing First, and help clients access housing resources across a broad KY region, while upholding our mission and values.

Qualifications

• A bachelor’s degree in social work, sociology, psychology, counseling, nursing, or related field is preferred.
• Two years or more of case management experience will be considered.
• Experience in a nonprofit organization or service-driven environment is valued.
• 100% commitment to Matthew 25's mission, vision and values.
• Flexibility to travel across Kentucky.
• Valid driver’s license with at least 3 years driving experience and less than 2 moving violations in the past 3 years.
• Must pass a drug screen and background check.

• Follow HOME TBRA and HOPWA guidance given by Kentucky Housing Corporation and HUD.

Overview:

Provides medical case management services under the Kentucky HIV/AIDS Care Coordination Program (KHCCP), including social service assessments and connecting HIV+ individuals with needed services offered by Federal, State, and Local community programs.

KEY RESPONSIBILITIES

• Promote a Housing First approach with clients.
• Follow HOME TBRA and HOPWA guidance given by Kentucky Housing Corporation and HUD.

Build relationships with property managers and leasing agencies

QUALIFICATIONS

• A bachelor’s degree in social work, sociology, psychology, counseling, nursing, or related field is preferred. Candidates with case management experience of two years or more will also be considered as viable applicants.
• Experience working in a nonprofit organization or a strong desire to work in service driven environment.

• Flexibility, as our services are available to a large geographic area in the state of Kentucky, which may require some travel
• Valid Driver’s License with at least three (3) years driving experience and less than two (2) moving violations in the past three (3) years.
