Job summary

Matthew 25 is a mission-driven healthcare nonprofit in Indiana seeking an MSN-trained nurse to deliver patient-centered primary care across our service area, focusing on HIV/STI care, education, and community partnerships.

You will conduct assessments, develop treatment plans, educate patients, monitor progress, and participate in quality improvement while maintaining detailed medical records. Local travel and licensure in Indiana/Kentucky are required.

Qualifications

• MSN in Nursing required.
• Active Indiana or Kentucky nursing license and national certification.

Responsibilities

• Conduct thorough patient assessments, including medical history reviews, physical examinations, and symptom evaluations.
• Develop and implement patient-centered treatment plans, which may involve prescribing medications, therapies, and other interventions.
• Educate patients about prescribed treatments and expected outcomes.
• Monitor patient progress through regular follow-ups and adjust treatment plans as necessary.
• Conduct screenings, immunizations, and health assessments to prevent illnesses and promote wellness.
• Maintain accurate and detailed medical records, including histories, progress notes, and treatment plans.
• Engage in ongoing professional development to stay current with advancements in healthcare practices.
• Participate in quality improvement initiatives and promote evidence-based practice.

QUALIFICATIONS

• MSN in Nursing required.
• Maintain a current and valid M.D., D.O., P.A. or N.P. Kentucky and/or Indiana license, along with national certification.
• 2 – 3 years family practice or primary care.
• Experience working in a nonprofit organization or a strong desire to work in service driven environment.

• Flexibility, as our services are available to a large geographic area in the state of Kentucky, which may require some travel
• Valid Driver’s License with at least three (3) years driving experience and less than two (2) moving violations in the past three (3) years.
• Must be able to pass a drug screen and background test.
