Job summary

Matthew 25 is seeking a dedicated social services professional to provide medical case management and connect HIV+ individuals with needed services across Kentucky under KHCCP.

The role emphasizes housing programs coordination, a Housing First approach, and collaboration with partners to support clients. A bachelor’s degree in social work or related field is preferred, with nonprofit experience and willingness to travel across the state.

Qualifications

• Bachelor’s degree preferred; candidates with two years case management experience will be considered.
• Experience in nonprofit or service-driven environment is desirable.

• Valid Driver’s License with at least three years driving experience and fewer than two moving violations in the past three years.
• Must pass a drug screen and background check.

Overview:

Provides medical case management services under the Kentucky HIV/AIDS Care Coordination Program (KHCCP), including social service assessments and connecting HIV+ individuals with needed services offered by Federal, State, and Local community programs.

KEY RESPONSIBILITIES

• Assist Housing Coordinator with Housing programs.
• Promote a Housing First approach with clients.
• Follow HOME TBRA and HOPWA guidance given by Kentucky Housing Corporation and HUD.

Build relationships with property managers and leasing agencies

QUALIFICATIONS

• A bachelor’s degree in social work, sociology, psychology, counseling, nursing, or related field is preferred. Candidates with case management experience of two years or more will also be considered as viable applicants.
• Experience working in a nonprofit organization or a strong desire to work in service‑driven environment.

• Flexibility, as our services are available to a large geographic area in the state of Kentucky, which may require some travel
• Valid Driver’s License with at least three (3) years driving experience and less than two (2) moving violations in the past three (3) years.
