Job summary

UNC Health Nash is seeking a dedicated social worker for Case Management to provide psychosocial assessment, care planning, and discharge planning for patients in the Emergency Department setting. You will coordinate with the ED care team, document progress, and ensure appropriate referrals and resources are available.

Responsibilities include crisis intervention, family advocacy, and collaboration with community resources to facilitate safe discharges and transitions.

Qualifications

• Minimum of two years paid experience in social work.
• Experience in clinical/medical setting and crisis intervention preferred.

Responsibilities

• Prioritization of Patients and collaboration with ED Care Team.
• Discharge and Transitions Planning with care options.
• Crisis Intervention and collaboration with agencies.
• Address Frequent ED Utilization with care plans.
• Communicates with team members and patients, ensuring confidentiality.
• Demonstrates knowledge of age-specific treatment and quality improvement.
• Performs other duties as assigned.
