Job summary

UNC Health Nash is seeking skilled social work professionals to provide psychosocial assessments and care planning for patients across Nash, Edgecombe, Halifax and Wilson counties. You will document plans and progress, coordinate discharge needs with ED staff, and assist with crisis intervention and community referrals.

Qualifications

  • Bachelor’s Degree in Social Work required.
  • Master’s Degree in Social Work preferred.
  • Minimum of two years paid experience in social work.
  • Valid NC Driver’s License, BLS and CPI required.

Responsibilities

Job description

Summary

Provides psychosocial assessment, care planning, intervention and evaluation to individuals and families undergoing treatment for patients and provides appropriate documentation of plan of care as well as progress notes documenting the patient’s response to interventions. Utilizes individual or family treatment modalities and makes appropriate referrals for community agencies and support services.

Responsibilities
  • Prioritize patients needing psychosocial assessment/interventions and address immediate discharge needs, documenting interventions and progress notes. Escalate barriers according to established procedures.
  • Discharge and Transitions Planning: Discuss options with ED staff, patients, and families to ensure safe discharge and coordinate post‑service needs, including community resources and psychiatric/addiction services. Collaborate with primary care physicians and other care team members.
  • Crisis Intervention: Advocate for patients and families, address crisis situations (e.g., violence, abuse, death, or sexual assault), provide counseling, and follow established protocols for protective services.
  • Address Frequent ED Utilization: Develop care plans for high‑frequency users, assess reasons for ED use, educate patients, and coordinate resources to secure a primary care provider.
  • Communicate with team members, physicians, and other care providers, giving concise reports on patient progress and ensuring confidentiality.
  • Communicate with patients/representatives, explaining treatment purpose and addressing concerns, while following grievance procedures.
  • Demonstrate age‑appropriate treatment knowledge for pediatric, adolescent, adult, and geriatric patients.
  • Participate in quality assurance and improvement activities, contributing data, analysis, and ideas to enhance patient outcomes.
  • Perform other duties as assigned, such as assisting with behavioral health housing placements and community liaison projects.
Education
  • Bachelor’s Degree in Social Work
  • Master’s Degree in Social Work preferred
Licensure / Certification
  • Valid NC Driver’s License
  • BLS required
  • CPI required
Professional Experience
  • Minimum of two years paid experience in social work
  • Experience in clinical/medical setting, crisis intervention, and work with groups, families, and individuals preferred
