Qualifications

• Bachelor's Degree in Social Work required.
• Master's Degree in Social Work preferred.
• Minimum two years paid experience in social work; crisis intervention experience preferred.

Responsibilities

• Prioritize and coordinate psychosocial assessment and intervention within the ED care team.
• Plan and document discharge and transition needs with patient and family.
• Provide crisis intervention and connect to community resources as needed.
• Develop and maintain care plans for high-frequency ED users.
• Communicate patient progress with physicians, nurses and care teams.
• Explain treatment purpose to patients and families to minimize anxiety.
• Assess patients across age groups and tailor interventions appropriately.
• Participate in quality assurance and CQI activities.
• Support behavioral health housing placements and related departmental projects.
