The role coordinates care from pre-admission to post-discharge, collaborates with healthcare teams and community agencies, advocates for patients, and contributes to care plans that improve patient flow and outcomes.

Qualifications

• Bachelor’s Degree in Nursing or Social Work required
• RN license in Minnesota required
• BLS certification within six weeks for social workers

Responsibilities

• Coordinate care from pre-admission to post-discharge and facilitate communication among healthcare teams and community agencies.
• Advocate for patients and develop care plans with appropriate referrals and resources.
• Support discharge planning and evaluate outcomes based on clinical and utilization data
